ABSTRACT

La Federación Nacional de Neurocirugía (FNNC) fue fundada en la ciudad de Rosario el 23 de agosto de 2019, con el objetivo de crear una organización que nu-clee a todas las sociedades y asociaciones de neurociru-gía de nuestro país, con un perfil netamente gremial. La FNNC reconoce a la Asociación Argentina de Neuroci-rugía (AANC) como la organización que rige la activi-dad académica-profesional de la neurocirugía en todo el territorio de la República Argentina.Uno de los propósitos principales es lograr condiciones laborales dignas y correctamente remuneradas para to-dos los neurocirujanos que residan en suelo argentino. Pensamos que una de las herramientas necesarias para conseguir dicho propósito es lograr un equilibrio entre el número de neurocirujanos en actividad, el número de plazas de residentes, y el número de habitantes en Ar-gentina.Actualmente no existen datos concretos para realizar un diagnóstico de situación. Es por ello que la FNNC, apoyada y avalada por la AANC, tomó la iniciativa de realizar un análisis descriptivo de la situación actual y su proyección en los próximos años en relación con el nú-mero de neurocirujanos en Argentina.El objetivo del presente trabajo es presentar de forma sintética y clara los aspectos más relevantes de dicho es-tudio.

ABSTRACT

Abstract INTRODUCTION: Among patients with Chagas disease, men have a higher risk of worse pathological symptoms than women. We aimed to explore the role of the Y chromosome in men diagnosed with Chagas disease and assess the relationship between their ancestry and disease status. METHODS In this comparative study, we analyzed 150 men with unrelated non-chagasic disease (nCD) and 150 men with unrelated chagasic disease (CD). We assessed the serological diagnosis of Chagas disease, biochemical parameters, thoracic X-rays, electrocardiogram, and transthoracic echocardiography and determined the haplogroup by analyzing a set of 17 microsatellites from the Y chromosome. We examined the associations between common Y chromosome haplogroups and the clinical parameters of risk by logistic regression. RESULTS For all patients, the most common haplogroups were R1b (43%), G2a (9%), and E1b1b (9%). The R1b and G2a haplogroup was more frequent in men with nCD and CD, respectively. As expected, we observed a high proportion of symptomatic patients in the CD group independent of the haplogroups. Men from both groups classified as having the R1b haplogroup showed less clinical evidence of disease. Multivariate analysis showed that CD patients without R1b were about five times more likely to have a cardio-thorax index >0.5% (OR [odds ratio] = 5.1, 95% CI [confidence interval] = 3.31-8.17). Men without the R1b haplogroup were 2.5 times more likely to show EcoCG alterations (OR = 2.50, 95% CI = 0.16-3.94). CONCLUSIONS: Our results provided evidence that the R1b haplogroup may have a potential protective cardiovascular effect for its carriers.

ABSTRACT

Aims: Endothelin-1 (ET-1) is a potent vasoconstrictive peptide, and its activity is mediated by the type A receptor (EDNRA). This action may play a significant role in the etiology of hypertension. There are different works that shows an association between certain polymorphisms of endothelin axis and clinical phenotype of hypertension. We describe the genetic variability +138/ex1 insertion/deletion (I/D) adenosine (A) in the ET-1 gene and polymorphism thymidine/cytosine (T/C) His323His in the EDNRA gene associated at the clinical variability in hypertensive patients. Study Design: Observational, transversal and analytical study. Place and Duration of Study: Hypertension Service at the Internal Medicine Department of Córdoba Hospital, and Biochemical and Molecular Biology Department in School of Medicine, National University of Cordoba, Argentine. Patients considered hypertensive between April 2009 and April 2010. Methodology: Were assessed 136 patients serum lipid profiles, renal and hepatic functions and were taken Thoracic X-rays, electrocardiograms, and echocardiographs. DNA extracted from circulating leukocyte were used to analyze the polymorphisms of genes by PCR-RFLP. Results: For the polymorphisms of Receptor A from Endothelin -1 studied the presence of cytosine homozygous genotype was less frequent in males (P = .02). For both genders, the same genotype was associated to low plasma alkaline phosphatase activity and cholesterol levels. The presence of thymidine nucleotide allele correlated with plasma alkaline phosphatase activity and cholesterol levels. The Thymidine allele correlated with the degree of cardiovascular compromise (r = 0.54, P= .002). For the genetic variant in the ET-1 gene, the homozygous adenine deletion was associated to normal plasma levels of glutamate/pyruvate transaminase enzyme activity, uric acid concentration, cholesterol, and Low Density Lipoprotein in hypertensive subjects without clinical risk. Conclusion: We observed a gender-specific protective effect for EDNRA gene variations, the subjects that carried the TT genotype presented more aggressive symptomatology. These results show an association between plasmatic biochemical parameters, the clinical condition, and polymorphisms in the endothelin axis genes.

ABSTRACT

Clear cell renal carcinoma is the most frequent type of renal carcinoma. Recently, attention has been focused in the expression of angiogenic factors by these tumors, which would justify in part their capacity to grow, invade and disseminate, stating a worse evolution of those patients with an unfavorable angiogenic profile. 83 samples of nephrectomy with a diagnosis of clear cell renal cell carcinoma were studied. Clinical and pathological data were collected. Tumors were studied to assess immunohistochemical expression of the following markers: VEGF-A, HIF-1α, CD34 and Ki67. Results indicated a direct linear relationship between expressions of these four markers. Besides, the expression of HIF-1α was directly related to Furhman grade, invasion of the renal vein and tumor stage. Likewise, tumor proliferation index, assessed with Ki67, was directly related to the presence of necrosis, capsular invasion and advanced tumor stage. Regarding the expression of CD34, vascular density was inversely related to tumor necrosis and overall survival. These findings are controversial compared with the available literature. Then, a research scenery would be open, where the importance of generating prospective and more standardized studies are highlighted to determine the role of these angiogenic factors in tumor evolution and prognostic evaluation of these tumors.

ABSTRACT

El estado nutricional de los adultos mayores es la resultante de sus condiciones de vida y de salud. El aumento demográfico de este segmento de población es un reto a los profesionales de la salud, para lograr conocer cuales son las características de normalidad en esta etapa de la vida. Objetivo general: Conocer el estado nutricional de la población de adultos mayores beneficiarios del programa ASOMA, e identificar desde un enfoque cualitativo, sus patrones alimentarios. Material y métodos:. Estudio descriptivo, correlacional, transversal y de campo. Universo: 10214 ancianos beneficiarios de tres programas alimentarios. Muestreo: aleatorio estratificado. Tamaño de la muestra: 387 ancianos. Respetando la proporcionalidad del universo, Programa ASOMA n=162. Técnicas: Observacionales y medicionales (antropométricas). Ecuaciones: Chumlea para estimar talla a partir de la altura de la rodilla e Indice de Masa Corporal IMC. Conclusiones: El grupo estudiado (n=162) estuvo representado en sus 2/3 partes por el sexo femenino (n=108), perteneciendo en su mayoría a la categoría de viejos-jóvenes (personas de 60- 69 años de edad), con características de pobreza estructural en sus indicadores socioeconómicos: bajo nivel de escolaridad (analfabetismo funcional: 75 por ciento), ingresos temporarios, por debajo de $ 200 mensuales, provenientes en la mitad de los casos de la ayuda familiar. La cuarta parte de los ancianos aún trabaja. Más de la mitad de los ancianos eran propietarios de la vivienda que habitaban, que compartian en el 86 por ciento de los casos con sus familiares. Los alimentos básicos de la dieta eran predominantemente energéticos (pan, azúcar y aceite), y los alimentos de consumo frecuente fueron el arroz y la carne de pollo. Vegetales de los tres grupos, la carne de vaca y frutas del grupo "A" aparecen en sus dietas en menor proporción y frecuencia. El 83,9 por ciento de los ancianos tiene una dieta monótona (consumo semanal de £5 alimentos diferentes), recibidos en el bolsón alimentario del programa ASOMA. La media del IMC fue de 27,4 kg/m2 con un desvío estándar de 5,33, siendo los valores extremos encontrados de 16,20 y de 46,60. El 24,1 por ciento de la población se ubicó en valores de IMC que indican normalidad en el estado nutricional de los ancianos Los valores de IMC según sexo mostraron en hombres un agrupamiento de los datos alrededor de valores normales y colindantes (malnutrición leve por déficit y por exceso).
